1. When your dating, your getting to know each other. When your going out your together bf and gf.
2. Well you have to DTR dude. Define the relationship. Usually do it after 2-3 months or when you feel like the situation is getting to the weird zone. If you are asking this question you most likely have to DTR dude. Over Communicate
3. When you're seeing each other 'exclusively' (not seeing others), then you are 'going out'. Dating each other for a month, and seeing each other just about every day would also qualify as 'going out'. Either way you put it would be correct.
